Serenity Care PACE Gets Recognized for Commitment to Care Excellence

Serenity Care PACE is leading the way in making sure that every day, every older adult receives age-friendly health care.

In October 2022, Serenity Care PACE received prestigious recognition by the Institute for Healthcare Improvement as a Level II Age-Friendly Health System – Committed to Care Excellence.

The four essential elements of an Age-Friendly Health System are known as the 4Ms:

  • What Matters: Know and align care with each older adult’s specific health outcome goals and care preferences including, but not limited to, end-of-life care, and across settings of care.
  • Medication: If medication is necessary, use Age-Friendly medication that do not interfere with What Matters to the older adult, Mobility, or Mentation across settings of care.
  • Mentation: Prevent, identify, treat, and manage dementia, depression, and delirium across settings of care.
  • Mobility: Ensure that older adults move safely every day in order to maintain function and do What Matters.

    • Starting July 2023! Participants meet 1x month with our Dietician.

Footcare by Nurses provides in home limb assessments, assess shoes and socks and gait, trim toenails, reduce toenail thickness, reduce calluses, remove corns, address fungal toenails and skin issues, help with edema control, perform a circulation-enhancing massage, reset balance and fit some shoes for diabetics and hard to fit feet. See a nurse for more info!

  • Mock audit complete March 2023
  • PHE ending May 11, 2023: Updates to policy and procedure when guidance is updated
  • Inservices 2023: Next in-service is 4/27/23 infection control and QAPI
  • ADH Participant Exercise Group: starting late spring 2023, stay tuned for updates

As part of the Age-Friendly Health Systems movement, we are currently one of only three out of 144 PACE organizations in the entire country to achieve the Institute for Healthcare Improvements prestigious recognition as a Level II Age-Friendly Health System – Committed to Care Excellence. For the year 2023, Serenity Care will continue to expand our age friendly health system by focusing on high risk pharmacotherapy to increase safety for all participants.
