Founded in 2014 as a non-profit 501(c)(3) organization, Serenity Care PACE provides compassionate and high-quality care, preserving the dignity and independence of frail elders living in the community. Serenity Care PACE is dedicated to delivering the highest quality of health care and wellness services to our participants. We honor the individuality of all ethnicities, national origins, religions, genders, mental or physical abilities, sexual orientations, and sources of payment.

Serenity Care PACE listens to our participants, communities and team members to identify opportunities to improve the lives of those we serve.
A team of health care professionals from several disciplines gets together to provide a mosaic of services tailored for each individual participant’s needs. We call this the PACE Interdisciplinary Team (IDT).
The team consists of primary care doctors, nurse practitioners, registered nurses, social workers, therapists, aides, adult day program professionals, transportation and home care coordinators, and dieticians.
The Interdisciplinary Team works with the participant and their family and caregivers if available, to establish a Care Plan without the limitations or rigidity of a traditional health insurance plan.
