What is PACE?

PACE (Program of All-Inclusive Care for the Elderly) is a national model of care that provides older adults and their caregivers health care and community support. Programs of All-Inclusive Care for the Elderly (PACE) are innovative because they provide continuous care and services, offering individuals eligible for nursing home care the option of continuing to live in the community.

The PACE model is based on the belief and experience that older adults with chronic care needs are better served living in their homes and communities wherever possible.

Serenity Care PACE’s Program of All-Inclusive Care for the Elderly (PACE) services and benefits include, but are not limited to, all Medicaid and Medicare covered services:

  • Adult day care
  • Dentistry & Vision
  • Emergency services
  • Home care
  • Hospital care
  • Laboratory/x-ray services
  • Meals
  • Medical specialty services
  • Nursing home care
  • Nutritional counseling
  • Occupational therapy
  • Physical therapy
  • Prescription drugs
  • Primary care (including doctor & nursing services)
  • Recreational therapy
  • Social services
  • Social work counseling
  • Transportation

PACE also includes all other services determined necessary by a team of health professionals to improve and maintain an individual’s health. PACE programs provide services in an adult day health center or the participant’s (enrollee’s) assisted living, supportive housing, or private residence in accordance with the participant’s needs.

Upon enrollment participants must receive all health care, primary care and specialist physician services – other than emergency services – as authorized by PACE or be fully and personally liable for costs of unauthorized services.

Serenity Care PACE’s PACE Center is available to all our participants in Hampden County, Hampshire County and Franklin County. Our Center houses a medical clinic, health professionals, a day program, and a support team to help support your care and wellness needs.
