ABOUT OUR PROGRAM

Welcome Provider Network Partners

  • Serenity Care PACE welcomes medical and health care providers, senior housing and assisted living centers, and wellness partners to join our network and provide services for our participants, where appropriate.
  • Serenity Care PACE, Inc. is a CMS and MassHealth/EOHHS contracted Program of Program of All-Inclusive Care for the Elderly (PACE). PACE is both a care provider and a payor of services that acts as the participant’s insurer.
  • The PACE model serves individuals over the age of 55, with complex medical conditions that put them at risk for nursing facility placement, but who, with PACE supports, are able to remain in the community.
  • The goal of the program is to provide the services and supports necessary to prevent unnecessary hospitalizations and nursing home placements by providing timely care at the participant’s residence.
  • By partnering with Serenity Care PACE, your organization may participate in this unique model of care to better the lives of the Participants and to grow your organization’s reach.
  • The goal of our Provider Relations Team is to streamline the care and services provided by our Partners to improve the lives of our Participants.
Our Community Partners

Serenity Care PACE is available in all of Hampden County, Hampshire County, and the towns of Greenfield, Shelburne Falls, and Turners Falls in Franklin County.
