Serenity Care PACE’s PACE Center is available to all our participants and houses a medical clinic, health professionals, a day program, and a support team to help support your care and wellness needs.  Serenity Care PACE’s PACE Center is easily accessible via PACE supplied transportation vehicles, personal vehicles or public transportation.

Serenity Care PACE is available in all of Hampden County, Hampshire County, and the towns of Greenfield, Shelburne Falls, and Turners Falls in Franklin County.

WHAT TO EXPECT

We offer two types of programming to help you maintain your independence

Our Day Program
  • Provision of activities, meals, personal care and transportation with your preferences in mind.
  • The Day Program provides meaningful activities and dining programs to stimulate socialization and physical activity.
  • Once you attend the Day Program, you will have access to our health care team who can help maintain your independence.
Our Clinical Program
  • Personalized health care from an Interdisciplinary Team (IDT) of eleven (11) health and wellness professionals, led by your Primary Care Provider.
  • An individualized care plan, managed daily by members of our team who will see you in our PACE Center or at your residence as often as needed.
  • Our shared goal is to help you stay in your home and your community and as independent as possible for as long as possible.
