Non-Discrimination Policy
As a recipient of Federal financial assistance Serenity Care PACE, Inc. does not exclude, deny benefits to, or otherwise discriminate against any person on the ground of race, color, or national origin, creed, religion, or on the basis of handicap or disability or age, sex, sexual orientation, or military service in admission to, participation in, or receipt of the services and benefits under any of its programs and activities, whether carried out by Serenity Care PACE, Inc. directly or through a contractor or any other entity with which Serenity Care PACE, Inc. arranges to carry out its programs and activities.
This statement is in accordance with the provisions of Title VI of the Civil Right Act of 1964, Section 504 of the Rehabilitation Act of 1973, the Age Discrimination Act of 1975, and Regulations of the U.S. Department of Health and Human Services issued pursuant to these statues at Title 45 Code of Federal Regulations Parts 80, 84, and 91.
In addition, Serenity Care PACE, Inc. shall assist in arranging to receive referrals from and provide care to persons with communication disorders, including hearing and speech impairments. The services of interpreters shall be obtained for persons who communicate using sign language. Serenity Care PACE, Inc. will arrange for the lease of a Telecommunications Device for the Deaf (TDD) or Teletypewriter (TYY) or amplified telephone receiver from the local telephone company to facilitate Agency to client communication as needed. Serenity Care PACE, Inc. will coordinate and/or refer for any special devices or aides to facilitate effective and continuous twenty-four (24) hour communication at no cost to the patient, for as long as he/she remains on service.
Serenity Care PACE, Inc. also recognizes and respects the patient’s rights when there is a need to facilitate effective communication in a language or form understandable to the patient.
Cultural considerations for all patients is respected and observed. Where such considerations hamper the provision of care or treatment, staff will notify the Clinical Manager (or designee) and the physician in an effort to accommodate the patient.
Serenity Care PACE, Inc. will coordinate and/or refer for any special devices or other resources to facilitate effective and continuous twenty-four (24) hour communication, as needed, at no cost to the patient, for as long as he/she remains of service.
If Serenity Care PACE, Inc. is unable to provide written information in the language understandable to the patient an interpreter will be provided, as necessary.
